I hope he improves quickly. It's terrible not having a reason or answers . Some days I'm fine others not so worse if I raise my arms like if to wash the windows or conservatory roof
Your son's situation sounds like Catecholaminergic Polymorphic Ventricular Tachycardia (CPVT), a heart condition that affects children and young people. The heart appears normal at rest but has an irregular heart rate during strenuous exercise (such as marathon training). The most common cause of CPVT is a defective protein involved in making sure the heart beats at the right time.
At rest, this protein works fine but adrenaline (from exercise) causes it to play up and produce an irregular heart rate. This is not very common, which is probably why the doctor has not thought of it. It only presents itself with exercise but is a serious problem.
See if you can get the doctors to check for mutations in ryanodine receptor 2 (RyR2) and calsequestrin 2 (CASQ2), as these cause around 50% of cases. Doctors also usually carry out an 'exercise test' to monitor the heart rate during exercise.
The following website should help:

Your son's symptoms definitely sound like CPVT to me so I would get this checked out. I really hope you get it sorted soon.
