Going from 1500mg Gabapentin to (starting dosage) Pregabalin 75mg twice day. My RLS mostly affects me from 7pm through 1am. At what times would you take the two doses of Pregabalin?

You don't need to take it in 2 doses. Take it in 1 dose 1 to 2 hours before bedtime.

AI can get things wrong. Twice a day is for neurological pain, not RLS.

And going from 1500 mg to 150 mg of pregabalin is equivalent to going to 900 mg gabapentin. You will suffer from withdrawal symptoms doing that. Ask for 250 mg.

Thanks Sue, I did ask, And DR. insisted I start with 150mg, then increase after outcome. I am under VA care, that's my option.

Do you have enough gabapentin left that you can add to the pregabalin even though your doctor doesn't want you too. Otherwise prepare yourself for withdrawal symptoms which can include insomnia, headache, nausea, anxiety, diarrhea, flu-like symptoms, pain, excessive sweating, dizziness, confusion, and palpitations.

Good luck. Doctors can be so stupid at times.

As Sue says, no need to take pregabalin in split doses twice a day. That is for neuropathic pain.And she's right about the dose equivalence, although I recently read an article that states it is difficult to work out the correct dose equivalence because of the different ways both drugs are absorbed and excreted.

But, as you have no choice because your doctor will only give 150mg pregabalin, take it all in one dose at night and, if you struggle, go back to him to get the full 250mg.

I generally agree with the advice already provided. I switched from 1800 mg of gabapentin (taken in 3, 600 mg doses) to 300 mg of pregabalin with no transition (i.e 1 day). I had no issues with the transition. I take it all 1-2 hours before I attempt to fall asleep. I have PLM that primarily effects me between 9:00 pm and 3:00 am so its best for me to take it all at once in the evening. I do think I have some side effect, the main ones being daytime sleepiness and memory issues but at least I can sleep. Its an unfortunate tradeoff.

I recommend extreme caution using AI for any medical advice. These things are not really AI, but rather very complex text generators. The results of a query are often riddled with serious errors and even complete fabrications.
