Switching from Gabapentin to Horizant?

I'm currently taking 2400mg of Gabapentin, for my PLMs, and gradually reducing..my plan is to switch to Horizant/Gabapentin enacarbil.

Does anyone know how much Gabapentin I need to be taking, in order to switch to 600mg of Horizant?

I'm looking for a website that will give me this specific info and can't find one. I am going to come off it as it is not helping me one jot and am exhausted with very poor sleep. As I live in the UK I won't be paying directly for the medication, and I know it is expensive in the US. Has anyone had this experience at all?

Horizant is basically the same drug as gabapentin except it lasts 24 hours. You can switch directly from what you are on. If you don't have symptoms in the daytime, I would switch to pregabalin since you don't have to split the doses. You can switch directly. The equivalent is 400 mg and you can go up by 25 mg every couple of days to 600 maximum. But as I suggested before, you probably need a low dose opioid like buprenorphine.
