Hi Archie: a few days ago you were talking about coming off a recently prescribed ropinirole because of problems and were advised to try to get a prescription for pregabalin or gabapentin.

Ropinirole, pramipexole and rotigotine are all dopamine agonists: experts in the treatment of RLS no longer prescribe any of them as the first line treatment, because of the eventual likelihood of augmentation, recommending pregabalin or gabapentin instead.

Is your doctor suggesting combining pregabalin and pramipexole before trying pregabalin alone?

You can but please, please don't. You do not want to be on a dopamine agonist like ropinirole and pramipexole. Up to 70% of people will eventually suffer from augmentation and coming off it can be worse than coming off heroin. Put augmentation in the search box at the top of this page and read the horror stories. Here is one example: healthunlocked.com/rlsuk/po... You need to have your doctor read the Mayo Clinic Updated Algorithm on RLS at Https://mayoclinicproceedings.org/a...

Hi Archie ... the comments by Chris and Sue below are really important. However, personally, I am finding that Pregabalin alone does not work, even at the highest level of 450mg per night. The only thing that does work for me is Pregabalin plus Ropinirole (Requip). I only take 0.125mg per night with the Pregabalin, but without it, my RLS does not allow me to sleep.

I talked this over with Professor Walker at Queen Square Neurological Hospital. He said that, despite augmentation, he would have not concerns about me taking up to 1mg with the Pregabalin. I do not feel comfortable about that sort of level, but for me, the 0.125mg is a life saver.

I started on Ropinirole in 2007 and augmentation finally hit me a couple of years ago, so I am fully aware of the terrible effects that it can have. But while that 0.125mg saves my sanity, I will continue to take it.

Warm regards, Sally

Using pramipexole together with pregabalin may increase side effects such as dizziness, drowsiness, confusion, and difficulty concentrating. Some people, especially the elderly, may also experience impairment in thinking, judgment, and motor coordination. You should avoid or limit the use of alcohol while being treated with these medications. Also avoid activities requiring mental alertness such as driving or operating hazardous machinery until you know how the medications affect you. Talk to your doctor if you have any questions or concerns. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

I would use Horizant instead of pregabalin. Currently I am using Horizant with pramipexole.
