I'm not able to access it even if my Uni account. As the paper is part of a book, I can only buy the paper for $30. The Uni also doesn't have the book.

Thank you for the attempt. I do appreciate it, it’s chapter 5 of the book. The book would be fantastic and would probably cure my insomnia.

For 30 dollars, I may just buy it.

The article is in a journal. You can purchase the pdf online for 31GBP.

If you live in the UK you can go to a public library and may be able to get it for mush less than that.

Thanks as always Manerva, (Oregon, USA). I am familiar with DOI numbers. I was not familiar with Sci-Hub. I did buy the PDF, I can't afford (or don't think it wise decision, $243 for the book. (Amazon)... now I need chapter one.

You can read a lot of the book on Amazon "look inside" but not chapter 5!

I have a PDF of chapter 5 and it is very interesting. In fact I have the entire book now and I have read at least half of it. I find it very interesting. My next one will be Dr. Buchfurer's Clinician's Manual. What else can we do in the middle of the night?
