Tl dr from the article :

Patients who suffer from Restless legs syndrome complain of painful symptoms in the legs leading to sleep disturbances. The mechanisms for RLS are still not completely understood. We have shown that also the nerve cells supplying muscles in the leg are responsible and hereby additional drug treatments may be ahead targeting these nerve cells

I don’t see the breakthrough aspect of the paper. They are probably the first that looked at nerve cells other than brain cells. But that doesn’t make it a breakthrough. Wish some scientists would try to be a bit more factual.

"the nerve cells supplying muscles in the leg" is the breakthrough - this is precisely why some find relief with hot or cold treatments in the legs, yoga stretches, etc. The authors acknowledge the role of the CNS in RLS - it begins in the brain, but the end result - the uncontrollable movements - are due to an increased excitability of the nerve cells that supply the muscles in the leg and may well be a target for therapies. So, somewhere down the road, you may still have the Dopamine and Iron dysfunction in the brain but it won't result in the uncontrollable leg movements of RLS.
