I know how I feel about this (though many will differ). I've heard literally for decades that "most men die with prostate cancer, not FROM prostate cancer". Yeah, but as anyone who spends time here knows, PC is the second biggest cancer killer of men (after only lung ca). So I don't think a pollyanna attitude is particularly helpful.
Sure, I understand the impulse. You don't want to needlessly alarm people. But a balance has to be found--and as the furious backpedaling of the USPSTF after the 2012 position statement giving thumbs down to screening demonstrates.
There is so much talk about the "harms" of "overdiagnosis". To my mind, the harms are not from overdiagnosis, but from overTREATMENT.
I've heard this talk not only about prostate cancer, but also DCIS in breast cancer, and papillary thyroid cancer. Maybe the problem is that doctors don't have (or don't take) the time to talk to patients about what a prudent schedule of surveillance looks like.
There will always be more or less aggressive cancers, more or less lethal cancers. Calling them something else will not solve any problems.
