Interesting. Targeted+systematic biopsy seems to be the best.

More recent and MUCH larger trial (37,887 randomized men - entire male 50-60 yo population of Gothenburg, Sweden) proved that PSA> 3 + mpMRI-targeted only is better at finding significant PCa while ignoring insignificant PCa:

I think it is still important to have a confirmatory mpMRI-targeted biopsy within a year.
