Did he have a biopsy? If not he should one. Treatment would depend on his age general health and weight.
Sounds like an MRI and a biopsy and see what the doc recommends.

It is most important to get the best imaging at this time. An in bore, real time, biopsy with the mp 3.0T MRI by someone with experience is best.
An accurate biopsy results in an effective treatment plan. If something is missed---?

It is past the time to continue watching and waiting.
The numbers (rising significantly) make it clear that it is time for testing and imaging.
A PSA above 9 is NOT normal.
There is time to act, but waiting now, doing nothing, is foolish IMO.

At that level, to get the most accurate results, see a qualified urologist and get a prostate biopsy. That's what I did at your level, and it revealed Gleason 7 (3+4), indicating treatment required (surgery or radiation). I chose surgery. Good luck and best wishes on a "negative" result.
Time for action, not continued surveillance. Get a different urologist or at least a second opinion if urologist isn't taking aggressive action. There is another option to surgery or radiation -- HIFU. Research it. Most urologists are not familiar with it and it isn't in their bag of tricks.

Another option is the just introduced TULSA PRO ultrasound treatment, now offered at several clinics in USA...one in Socal and 2 in SE USA. Problem....as of now, no insurance coverage , and runs approx $30k. also, longer-term results are still lacking, compared to data for surgery and radiation. Not zero, but significantly fewer side effects!! Leading Docs are talking about this as the future of low risk and intermediate risk standard treatment.
I am the friend and have an appointment with a different Dr. New Dr does not have 3T mri. That is what I was pushing for.
Where are you based ? It shouldn’t be hard to find a 3t mri. But equally important Is for you to not only have a doctor who is seasoned at reading them but also a specialist on the 3t mri who your doctor can trust will read it properly and give an accurate report. When there were questions raised about my 3t mri by two different doctors, I got a second opinion by a specialist at Cornell Weil in New York City who basically put everyone’s concerns at bay. What was considered a likely extra capsular extension was attributed to camera shake. This one seemingly minor thing virtually changed my whole treatment approach. I ended up going with HDR brachy monotherapy rather than a variety of dual treatments that were suggested based on the initial incorrect reading. In the end, get as many opinions as you can after doing the research. Then let your gut guide you to make the decision that’s right for you. Trust the process of doing research, meeting doctors and then being your own advocate. In the end you want to make sure, in a situation where there is rarely 100% certainty, that you are fully confident you have the best possible 3t mri along with the best possible reading of it. Here’s hoping you receive a negative on PC. But if it’s not, know there are a lot of great treatment options out there for you.
I am near Dubois Pa. Unfortunately I recently found out thru an employee of another Urologist that my Dr has a history of letting thing progress too far before taking action. I was pushing my Dr for the referral to State College but he was stalling me. I did not push had enough apparently. I now have and appointment with another Dr. but does not have the 3T. I recently started feeling "something" in both shoulders and my left hand. Some feelings of "needle pokes" in different areas as well. I will be seeing the new Dr in 1 week. The anxiety that is going with not knowing is overwhelming.
