I had a radical prostatectomy September 30, 2015. (DaVinci) PSA 10.0 Gleason score was 4+4=8. Today my PSA 0.4. However incontinence is still present. Not to bad, but not completely healed yet either, also impotence is a big problem. Any ideas on how to try to fix these two issues? Would like to hear from others experiences.

Kegel exercises have helped me with regular control except if I drink coffee and am traveling farther than 30 minutes. After surgery I was using a pump to get blood flowing into the penis. But could not do so on my own. I believe my nerve bundle did not survive the Davinci. Thus I am not able to have an erection. It has been 5 years. None of the pills work. But tried the shot and that gives an erection but for longer than what I desire. Make sure you talk to your urologist about all your options. I am sorry that your recovery is slow. I had a gym trainer help me strengthen the abs muscles due to the incisions. Those incisions were tight for years afterward.

First, I find a PSA of 0.4 post surgery from last September a little concerning and in need of some very active monitoring.  What has been the PSA history since the surgery? 

Both the incontinence and the impotence issues could continue to improve with some additional time. 

Have you been doing any penile re-rehabilitation to help the impotency issue?  If yes, what have you been doing?  This is really important.

How about Kiegel exercises to strengthen the pelvic wall?

PSA at 3 months  was 0.0 and at 5 months was 0.4. I will be tested again in a couple of weeks. Thanks

Tony,

Your PSA number is in fact very concerning.  With PSA undetectable at 3 months then 2 months later a PSA of 0.4 there is an excellent chance that you have had a recurrence of the cancer.  I urge you to not wait long before your next PSA test.  I also urge that you start thinking about what you might do if you do have a recurrence.

1,  The most alarming issue is the PSA indicating either an incorrect reading (should be undetectable if cancer free and not having another issue such as an infection).

2,  Incontinence issues...do lots of core exercises, not just Kegels.  You may never be the same but at least you will markedly improve.  Try Dr. Purohit 212-772-3900

3.  Impotence...If I were you, I would think either injections or implants down the road. Schedule a visit with Dr. John Mulhall at New York Hospital for sound advice.
