After seeing a GP or your Rheumatologist to examine your chest, you would normally have a chest X-ray, your Peak Flow would be measured plus a resting oxygen saturation level.

I’ve had my Chest X-rayed several times while being on MTX due to wheezing/coughs, MTX was never the cause.

I’m not sure if or what blood tests would show damage up.

Have you had an X-ray?

There’s a link in FAQ’s re Blood Tests - you could scroll through that to see if there’s any information.

If it is an infection that would raise ESR, CRP (in particular) and white cell counts. On the other hand, MTX-related toxicity causes inflammation and ESR and CRP are indicators of inflammation somewhere in the body. oO I suspect there isn't a simple answer.

But MTX can do it without an infection and that needs to be checked out as MrsN says. It can also lead to raised temperature which could be confusing I suppose.
