But he said you can only be on pred for 2-5 years, after that, it’s methotrexate because the pred symptoms are too severe?

Whatever he thinks - methotrexate doesn't replace pred if you have PMR. If methotrexate works on its own, it was probably misdiagnosed in the first place.

It's also a sweeping generality. Most people get down to a physiological dose (about 7.5mg and below ) for longer term use and that isn't going to cause much trouble for most people:

At low doses the so-called pred effects are also found in an age-matched patient population who aren't on pred and don't have PMR. I've been on pred for well over 11 years with no identifiable problems - and had a lot of so-called pred adverse effects with PMR and no pred in the first 5 years. And when I tried mtx I developed so-called pred effects that I had never had with pred on its own - mtx not only supposedly increases the pred antiinflammatory effect, it also enhances adverse effects.
