I’m struggling with pains again at the moment. I’m not sure if I am at the start of another flare. I’ve been ok since my recent taper from 8mg down to 7.5mg preds, I seem to be getting some aches in my upper arms and upper thighs. I’m not sure if this is DOMS...... I have been working more than usual over the past couple of weeks which has involved several trips up and down a ladder.

Is the any way of telling the difference between Doms and pmr pains??

DOMS usually starts about 24 hours after exercise (up&down ladder?) - and last up to 72 hours. It can be treated with an Epsom salts bath.

If it lasted longer than that, could be a flare.

Really think you have answered your own question: "I have been working more than usual over the past couple of weeks which has involved several trips up and down a ladder."

Normally DOMS will last a few days, as DL says, but when you have PMR in the first place there are other factors in play. Your muscles are unable to signal to you that they have had enough so you carry on doing things past your limit. If you then continue to use those muscles despite the damage already done you hold back the healing process even further than the PMR already does so it may be more than a week or even longer that you can feel it!

Using ladders requires arm involvement and it is climbing steps the hard way - that would account for upper arm and thigh pain.
