This is day 4on 20mg pred ,feel disappointed that I don't feel much better. But must be a little as I haven't needed a heat patch on shoulders ,but was expecting to be much better
Am I expecting to much to soon ? Do you feel painfree all day after taking pred or does PR still come back at night .

Hello. I’d say give it a bit more time before losing hope. I know people say it can be a magical transformation that works in hours, but that’s not everyone; you’re not even a week in yet. Just one question, what are you doing in your daily activity?
Did mow the lawn yesterday, no one else to cut it , as I live alone!! And haven't done it for a couple of weeks. Could that be the reason. I was trying to ignore things.
I felt great after a couple doses of prednisolone (nb not everyone does) but overdid things -including cutting the lawn-more the emptying/lifting of the full grass container was the problem - and complained to my GP after the weekend that initially ok but not sustained improvement-led to me being bumped up from 20mg to 40mg a day which I probably didn’t really need. It’s very difficult not to overdo things but I really think resting is the absolutely best thing you can do in the early stages. All good wishes.
I did find that Pred wore off at night and I felt a lot better in bed. It is early days yet. You need to work with it though and appreciate that you are still ill. Pred only deals with the inflammation while the disease rumbles on. Sorry that you feel anxious again. Let yourself be poorly - cherish yourself a bit. X
It can take a few days for some people to feel the full benefit and some people need more pred than others. Give it a few more days and then discuss with your gp if no better.
Try this..................take your pred 20mg, as soon after 2-3am as possible with a cup of tea and a slice of bread, never on on empty stomach. Then snuggle down back to sleep. Then when you wake up a warm shower, warm not hot. No soap etc necessary just strips natural oils from your skin.
You might find this makes you feel better.
Cutting a lawn at this early stage is not going to be easy, hope you did at for 5 mins, rest for 10mins etc. The oxgen supply to your muscles is impaired so easy does everything.
Think you might be expecting too much too soon - and as others have said you still have to pace yourself - just because some of the pain has gone - the PMR is still very much alive.
If you are getting a return of pains in the evening then there are a few options why -
Maybe your dose is not enough to last 24hours to thereabouts,
Maybe your body is not getting the full benefit of the dose - some people don't, or
You are trying to do too much. You must remember you have a serious illness.,
Just to reiterate what I said in my link I sent to your previous post (please read it again) - the steroids are only addressing the inflammation caused by the PMR, they are doing nothing for the underlying illness.
Plus that inflammation is renewed everyday by the substances (cytokines) produced in your body every morning.
Sorry you are still having problems, but, yes, I guess you probably are expecting too much on the basis of some lucky people's experiences.
I spent three days on 20 mg, and was upped by my excellent GP to 30 mg as I still had pain. I stayed on 30 mg for three weeks before beginning my taper.
A lot of what Dorset Lady writes applied to me, insofar as the pred didn't last all day, and I guess I was most likely trying to do too much. We all, or nearly all, learn the hard way.
Gradually the pain did subside, but it wasn't an instant cure for me. 40mg does seem a bit over the top, but I am not a medic. At least it should knock your pain on the head assuming it's PMR you have.
I was never pain free , still aren’t , but I am definitely better. ( some days better than others). Really try and pace yourself , go slow, eat slow, drink slow, learn to play /work in the slow lane ( and move to middle lane when the time comes. Avoid fast lane all the times, even when better, your body and mind will thank you. Took me awhile to grasp and input, but it works !

Started 60 mg , tapered by 5 each month until 30 mg. Then by 2.5 till 15 (where I now). Will taper 1mg till 10. ( hopefully!!!)
Aches pains along the way , but rinsing with salt water, resting every morning, heating pad, low carbs , mediation and yoga, knowledge ( this site) and accepting this is the new me ( for now). It’s life debilitating but not life threatening! I also have a terrific team of doctors. Good luck to you on this journey. Take your time and be good to yourself❤️
Mine kicked in after a couple of hours, but if I did anything like cutting the lawn the sore/ache did come back. Also, for a few months, I still had pain in the morning until I took the next dose. I’ve tapered down to 4.5mgs and the pain has completely gone away no matter what I do. That in itself brings it’s own problems, it’s as though the PMR has completely gone so it’s a big temptation to taper quickly. I have to keep reminding myself ‘slowly does it’. I know you say you live alone but go easy your health is more important than a neat lawn. Good luck, things do get better.
