It is cytokines released into the bloodstream which cause inflammation.

The substances that cause it are cytokines - there are quite a few of them and are normally occuring substances in the body with a pro-inflammatory action that are shed regularly at specific times of the day. When they function normally they help healing and other processes. When the processes go wrong - you get excessive inflammation of various types.
