Cytoreduction is a long-term treatment for ET. Hopefully you will find you can tolerate the PEG long-term. Many of us do well on it. Note that tolerance is dose-dependant.
The symptom you describe in your arm could be from the ET or from a PEG adverse effect. It sounds like one of several possibilities: paresthesia, peripheral neuropathy, some kind of microvascular symptom, or something else entirely. I would definitely not ignore this symptom. It would be advisable to let your care team know ASAP.
Please let us know how you get on.
