I am applying to be a Personal Welfare Deputy for my son. I have to send copies of my application to all NHS bodies involved in his care and treatment.
It's straightforward to contact his GP and some others but I need to write to two hospital Trusts and in these cases I don't know who to address these papers to because I have never met the staff concerned.
With the hospital I wondered about sending the documents to the lead learning disability nurse. The other Trust covers dentistry so I have asked his supported living provider if he sees an individual dentist who could be contacted.
I do wonder with these large NHS bodies if it's better to write to someone in the hospital hierarchy, which would cover aspects of his care under various departments, but I can't pinpoint who would be suitable.
Can anyone advise me about this?

Hi speak to medical records and you can take a copy in and they can add it to their system and flag you as a deputy that’s what I’ve done and it works fine and I did the same with the g p surgery as well
Hi. Have you phoned the helpline (is it the office of the public guardian helpline?) They will be able to advise you specifically who you need to send your application to. I think the point of this part of the application process is to give other professionals the chance to say if they dont think you are suitable to be the deputy, so it may be that just your GP plus social worker is sufficient. Jofisher above has misunderstood your question I think, and is referring to whom you give a copy of the deputyship order to once it has been approved.
