Have you or someone you support had to go to hospital during the coronavirus pandemic?

Hi everyone

We are helping Healthwatch with some research they are doing looking coming home from hospital (discharge) during the coronavirus pandemic.

If you or someone you support has a learning disability, and has had to be admitted to hospital during the coronavirus pandemic (since 1st March 2020), we would love to hear from you about your experiences.

The surveys will be up for 1 week only - until Thursday the 20th of March.
