I would first trnebulizer treatment, if that doesn't help or if he doesn't have nebulizer and the albuterol inhaler isn't working, go to the ER.

Try to keep him com when he is having an attack , helping him with his breathing in his nose and out of his mouth. Trying to take his mind off how bad it is.
