Hi JBux,
My family are in much the same situation, we are also struggling with continuing care since my husband was discharged from hospital after a traumatic brain injury and it has also affected my grown-up children. I have just posted a question about PA's/ Personal Care Assistants which may also be useful for you to look if others have experienced this.
I am sorry we can provide no useful information to you but I hope it helps to know that you are not the only one in your position.
