The last two questions on the v1 medical form
The second to last question is about double vision. If you do have double vision you will need to answer those questions. If you don't then tick no and go to Q4. Q4 is referring to the minimum standards you need to meet when driving. You must not have been told your vision is worse than 6/12 on the snellen chart (with the letters that you read by the line that get smaller as you go down the chart). You also need to be able to read a number plate from 20 meters. If you are unsure, speak to your eye specialist or speak to an optician who would be able to advise you on this as well.
