I faint and fall unconscious for a few minutes randomly. I thought that it happens when I take Pramiprexole, as it happened twice earlier in that context. It usually happens after C/L.
I feel nauseous and faint. When I regain consciousness I vomit.
This is not due to any hypotension. I faint in sitting position.
Any ideas?

Could it be vertigo? Or neck dystonia? Or other neck issue? Or vagal neuropathy. I know my dopamine levels are lowest at the moment no when I take dopaminne. Dystonias are worse when dopamine is low. Neck dystonia can be hard to spot.
Also, re: vertigo that you mention - I have this. the room spins and whether standing or sitting plus nauseous. It has to do with crystals misplaced in your ear. I control by taking decongestants when I feel least bit congested. The real cure originally for me , until I discovered preventative congestion idea on my own, was the Epley maneuver (& there are other maneuvers ). Pretty easy fix if this is the problem! be sure someone drives you to the PT and be sure the PT drops your head below the table -->( look on line) Google of course. best of luck!

but it is not that simple
What you have described is classic sudden low blood pressure as caused by Parkinsons and parkinsons medications including LD. It doe not take a large drop in blood pressure to start things rolling.
Tips to help reduce low blood pressure episodes include:
Drink water and increase the amount you drink, Try sport 's drinks that contain electrolights
Minimize or avoid drinking alcohol
Get up slowly and stay still if feeling dizzy or lightheaded
Avoid sitting or laying flat for long periods
Avoid things that can cause you to overheat, like hot baths, saunas, or being outside for too long on a hot day ( Heat stroke and sun stroke are more prevalent in PD and it does not take much to push you over the top)
Raise the head of your bed slightly
Try to eat smaller, more frequent meals
Consider wearing compression stockings that reach all the way up to your hip or over your belly to avoid cutting off circulation to your legs or at minimum compression tubes over your feet and ankles , with loose diabetes socks with padded sole over the tubes and spongy sole inserts in your shoes .
I have done all these things every day with good result but before when the situation was like yours , tests and measurement showed nothing but there was something wrong (classic PD statement ) I suggest that if you treat it like momentary low blood pressure and follow the tips that the fainting spells will go away.

YES! - This was happening to my PWP - it is called syncope and pramipexole is notorious for it. How much are you taking? PWP ended up in hospital after he collapsed and knocked himself out. Then tried to get out of chair because he was going to vomit and fell against a table and it took me 1/2 hour to get him lucid and steady enough to get him off the floor.
And he fell in the shower and broke 2 ribs and knocked himself out.
Late last year we got him off it over 4 months and he has not had any issues since. Be aware it is not easy to get off it and it must be done slowly.

There are so many possible causes, some of them very serious, you need to get to an internist, internal medicine specialist.
Is it when you sit up or are you already sitting....Just curious, do you have a very slow heart beat that could require a pacemaker. Most likely the pramipexole, but just wanted to throw it out there.
Sorry to hear about your episode with pramipexole. As stated in the response below, Pramipexol can have effects as much as a year after the last dose. According to drugs.com. When I was first diagnosed with Parkinson's, the neurologist tried to put me on Pramapixel.. it made me extremely dizzy. and Sleepy.. I looked. it up and realized that many people have fallen asleep like you said without. warning, regardless of what they were doing, even while they were driving a car.. This caused one woman to have two serious accidents that cost her her driver's license. I personally think that it's irresponsible in the part of a neurologist to prescribe this drug without. making people who understand the side effects of it on the front end so they can be prepared for them. And of an agreement with the treatment program..
I had short blackouts and had fuzzy buzzing in my head for about a year. Stopped driving and saw several specialist. Was tested for mini strokes and passed. Cardiologist put me on a monitor and it turned out my heart was stopping for several seconds. Pacemaker cured all the problems
My mother had what we thought were fainting spells, but it turned out that she needed a pacemaker. Her heart would stop briefly, and it would look like she passed out.
Hi, my husband has PD. He had trouble with falling and something he said about how he was feeling made me think of electrolytes. I gave him some potassium (Dr Berg's brand) that I happen to have on hand. Then I read that PD peeps have a deficiency in potassium. I make sure he takes some every other day and include avocados with dinner. He hasn't had a problem with falling since then - worth a try-
oh testing for potassium - google it - there's a much truer test than usual one the doctors prescribe. We didn't test him though, just use the powder potassium and avocados.

Buenos Dias JayPwP, yo tomo Pramipexol de 0.5 Tres veces al dia (mañana, mediodia y noche) y Gracias a Dios nunca he tenido efectos secundarios, llevo 2 y medio años tomandolo, junto con Amantadina, tomo L/C 100/25 media tableta 8am, 12am, 4pm y 8 pm, Realizo 1 1/2 hr diario por la mañana ejercicios de fuerza, equilibrio y cardio, creo que eso me ha ayudado bastante en la ralentizacion de la EP, a lo mejor lo tuyo es como describe Edward Hopf, checa con un cardiologo para que puedas estar tranquilo con ese episodio del desmayo, que todo salga muy bien.
