I tried one offered to me by the Mpls VA and didn't take it because it is a crude device playing loud, obnoxious noise that forces you to talk over it.

yeah I just went to investigate this and that seems like it could be an apt description- plus not every person with speech difficulties has hearing difficulties and loud noise can be a real irritant - I think the LVST is somewhat helpful but maybe it is frustrating for some - there is a video on youtube to do this without leaving home
