Actually given the source of many cases of PD is possibly due to exposures to toxins or diet or viral or bacterial infections or lack of exercise or stress etc wouldn’t it be better to have a program of eliminating any potential causes before this procedure is done. Then perhaps the effectiveness might not wane so long as the lifestyle changes are maintained and toxins and infections aren’t reintroduced?

You do it when you need it. Delaying it on the (fundamentally speculative) basis that you have described here is probably just an efficient method of sacrificing good life years, frankly.

There's no compelling evidence (and by that I mean, a pattern of success) that proactive 'toxin elimination' has an impact on the pathogenesis of PD. There's every chance that the toxins that wore down your brains ability to protect against permanent neuronal death are long gone anyway.
