During the past 3 years I have been consistently reducing weights in the gym along with increased fatigue and weakness. I have noted this seems to be related to the intake of levodopa, because at evening/ night when not medicated I feel alert and more strength but then suffer insomnia. I’m on my 15 years with PD.

Dr. Costantini had some success using HDT for diseases that had fatigue as a major symptom, well at least in terms of ameliorating the fatigue aspect.

Aside from that, you might give consideration to Acetyl L Carnitine (ALC or ALCAR).

If you are weight lifting, the ALCAR may be beneficial there, as you probably already know, for getting the most build out of your lifting.

If you take ld agonists (Pramipexole for example) talk to your neurologist asap
I’m just taking Sinemet and mucuna. I quit pramipexole and other dopamine agonist few years ago, but I’m curious to know what’s your concern. Thanks!
I require levodopa specifically for sleeping and can make do with less during the day. I suggest trying the same - an extended release formulation of levodopa for night time, and a bit less than your current dosage during the day. Of course consult your doctor and get permission for any medication changes.
