Sinemet CR and IR

6 times per day.I Ingest Sinemet CR + IR (either 1/2 or 1 25-100 IR).

I would like to ingest only CR at 6 pm and 10 pm and 2 am.

Is there away to do that with only CR?

6 pm: 1 CR + 1 IR.

10 pm: 1 CR + 1/2 IR.

2 am: 1 CR + 1/2 IR.

Oh, my CR is 50-200.

Not me.

Neurologist said take more often to avoid high and lows.

Pharmacist said not to cut CR tablet.

Carbidopa; Levodopa sustained-release tablets | Cleveland Clinic.

Excerpt: Follow the directions on the prescription label. Swallow whole. Do not crush or chew. You may cut the tablets in half. Take your doses at regular intervals. Do not take your medicine more often than directed.

Remember also that CR does not absorb as well. My doc said for every 100mg CR you only get 70mg.

I’m curious ...Why do you want to cut back on the IR? I’m similar to you I take Sinemet 6 -7 times a day and I mix it up a bit because sometimes I need to be sociable or sing in my music group. Today for example went like this:

8 AM 1/2 IR then 1 CR,

11:00 1 CR

2:30 1/2 IR and 1CR ,

5:30 1/2CR and 1/2 IR ,

8:30 1/2 IR

10:301 CR ,1/2 IR

3:00AM-4:30 ish - 1 CR ,1/2 IR

I’m probably a PIGD subtype And I was diagnosed in 2012 , October . I believe I have a lot of trouble absorbing my meds because of IBS-c.

Thank your sharing your dose schedule.

I have PPH and, after each meal, I develop severe cerebral hypotension. The episodes (during which levodopa doesn't cross BBB last from 2-3 hours, occasionally longer.

I would like to remove concerns about food/eating as a factor at night.

I still don’t understand why you want to cut out the IR Levadopa—I think I’m missing some logical step!( I haven’t had my morning coffee yet!)

Also ,I’m learning something here from you; I didn’t know that postprandial hypotension would prevent the levodopa from getting to the brain.

If you have a concern about food causing a lack of medication absorption, why not just eat twice a day? Say, 10 AM and 6PM? You would get a good sleep from the last dosing, presumably.

Food/eating timing and types of food all seem to affect my on times

And off times.. sometimes its instant off with some food or drinks..water seems to help absorbent foods like baked goods but products with flour oatmeal.. and too many peanut butter cups.... hurt. ALSO I would recommend the newly approved drug xadaago

Has helped me out ..the best amount of any drug is the least amount you can take with doctors permission.. good luck

I take only the CR version so using only CR is possible.

I read info about 50/200 CR is taken twice a day. 90 min to begin effect and ten hours duration. What is your protocol?

My protocol has varied over time. Currently taking one quarter of a 50/200 tablet every eight hours. Does not seem like much but it does make a difference and for me a higher dose is adverse.
