I take Sinemet every 2.5 hours, and during the night when tremor wakes me I take Sinemet and 1 sinemet ER (usually 2-3 times per night). Is it critical that I take my medication every day at the same time? (So need to put an alarm for night and alarm for the morning). Or it is good enough to take medications every 2.5 hours, as I have different times that I wake up in the morning. At times, I can wake anywhere between 7 - 9am. I am trying my make my schedule work. Because sometimes the drugs don't work at all, or may be too much when I have dystonia and dyskinesia.

Sinemet is for symptomatic relief so let your symptoms be your guide. If synchronizing timing to when you wake up works satisfactorily then you have your answer.
Are you aware that if you have levodopa and protein in your gut at the same time, the two combine and ceases to be of any use to the patient, because it does not get into the brain, where it is needed. It is useless saying that you have to have both food and levodopa, because you can't stop that happening.
The drug industry makes a lot of money out of you and you get very little benefit.
If you take levodopa within at least 90 minute of eating food containing protein or you eat protein withing at least 45 minutes of taking levodopa, you are just wasting your money.
You have to follow those rules to get proper value for your medication.
at my last meds change...with my Neuro, he had already given Sinemet "CR" slow release 200/50 . 2 @ bed time. I also was told about the 45 mins either side of meals. yes consult (doctor) at least, cos , i was having dreams and lashing out..while asleep..injuring wifey so got given sedatives which helped .Good sleep now .
My husband's Sinemet was wearing off way too soon so he is now on Rytary that releases slowly into the system. He takes it every 4 hours, on the dot, 5 times a day. The last dose before he goes to bed. He can also take regular Sinemet , 6 pills through out the day, a needed. It has helped him to do it this way. His new motion specialist prescribed it this way.
My husband had high blood pressure before Sinemet and since taking it, his blood pressure is lower and yes he has a fast pulse rate in general. Nothing to do with eating.
