Sharing this in case it can be helpful to anyone. I wrote to Dr Cosentini for advice on Thiamine dosage this morning. He responded in about an hour! Here it is:

Dear Dr Cosentini and company,

Hello from the United States! First, thank you for all that you do for people with Parkinsons Disease! I also have Parkinsons Disease and wish to consult you about the dose of Thiamine that I should be taking. I am 49 years old, 235 pounds (106.5 Kg). I was first diagnosed 5 years ago. My UPDRS score was 31 but came down to about 21 after 2 or 3 months of taking 4g per day of B1 Thiamine HCL. At that point I have stopped improving and I am wondering if I am at the correct dose. Will you please advise me on the dose for my situation?

Thank you so very much for the advice! One more question - How much coffee is OK? I havev heard that caffeine interferes with B1 absorption.

His reply:

I think a single cup in the morning, half an hour, an hour before taking the thiamine will not hurt you.

How has your bradykinesia fared via B1 as I know we both share this?

at first it was helping but the last 2 weeks or so it has been creeping back. Going to try altering my dose down as per the great Dr.

I also am trying Gabapentin, as advised by Baily_Texas and as per a study I googled which said it help with bradykinesia. I have been on it 2 months and have not found it to be much help. Maybe a mild reduction in my upper posterior leg pain.

Gabapentin for Parkinsonism: A Double-Blind, Placebo-controlled, Crossover Trial

Abstract

PURPOSE: Gabapentin is a recently available anticonvulsant whose mechanism of action remains unknown. We suspected efficacy from serendipitous observations of gabapentin in patients with parkinsonism. This led us to a double-blind, placebo-controlled, crossover trial.

PATIENTS AND METHODS: We administered gabapentin in a placebo-controlled, double-blind, crossover trial to 19 subjects with advanced parkinsonism. We measured the effect of placebo and gabapentin on subjects' symptoms with the Unified Parkinson's Disease Rating Scale, the Webster Scale, and the Hoehn and Yahr Scale. We assessed tremor with surface-recorded electromyography.

RESULTS: Total Unified Parkinson's Disease Rating Scale improved with gabapentin compared with placebo (P = 0.0005). Likewise, activities of daily living and examination subscore of the Unified Parkinson's Disease Rating Scale improved with gabapentin compared with placebo but did not achieve statistical significance. Webster Scale showed improvement but neither Hoehn and Yahr Scale nor Webster Scale changes reached statistical significance. Tremor as measured by the Unified Parkinson's Disease Rating Scale improved with gabapentin but the use of the root mean square of the rectified electromyography as a measure of tremor activity was not statistically significant.

CONCLUSIONS: This study demonstrates that gabapentin improves rigidity, bradykinesia, and tremor of parkinsonism including both Parkinson's disease and Parkinson's syndrome. The rigidity and bradykinesia of parkinsonism improve on the drug even when the effects of gabapentin on tremor are discounted

Not interested in exenetide or any traditional meds. By the way, im done with Gabapentin. No good results. Im done with Isradapine too. Just ithiamine and amino acids. Thats all i need! I do think its available in the usa in some form though.

I guess if cutting the dose in half, for me, to 2g, does not work, then I go the other way and increase to 5 or 6g.

No. I was taking 2g at a time. Now i take 1g twice a day

my experience: After starting in the 4g level of thiamin HCI, within a very short time and for the following fifteen week my bowel movements were regular and smooth. BIG change from the previous years. Moved to 6g a day to see if that level would affect tremor. Became constipated after two weeks. Also some symptoms started to return. I reduced back to 4g daily, now much improved. No constipation. All symptoms suppressed.

I was at 1g twice a day, but it wasn't doing anything and i got impatient. So now i take 1.5g twice a day and im improving again.

Video yourselves doing a Pull Test and do the video again in 6 months and 1 year. I am not a believer in the Dr. C miracle. I think some people have had some benefit especially those who are in the early stages of the illness, but my suspicion is that for those who are in the later stages the benefit will be short lived. I do not believe thiamine will stop progression. I believe it improves symptoms by healing the body at a cellular level. I have seen no scientific evidence to support a belief that thiamine attacks the root causes of PD.
