Hi I would contact your local council and ask for the Welfare Rights department, they filled out my form for me and got high rate mobility and middle rate care but I work with them so it was easy for me to get it done or you could always try the citizens advice bureau however they don't have as much success rate as welfare rights do as welfare rights will fight any appeals for you as well.
