With the development of targeted therapies, some CLL researchers wondered if beginning treatment earlier might be advantageous for a subset of those with CLL, i. e. those with poor markers/complex karyotype (bad cytogenetics). A few clinical trials were started to answer this question, but so far, the guidelines haven't CHANGED. That may happen eventually as treatments get more selective with less side effects, so keep across changes in these guidelines.

As I mentioned in an earlier reply to you, high CLL blood counts are not a trigger for starting treatment. It's the speed at which the lymphocyte count is doubling, with a doubling time of under 6 months, particularly when the lymphocyte count is over 30(,000) of concern.
