Dad Diagnosed with Decompensated Liver Disease aged 83

Childs Pugh B - MELD 11 Liver Cirrosis to NAFLD < This is what it says on his letter.

Im here primarily to ask, at what point should I call the hosptial for help?

My dad has severe ascites and other symptoms (fatigue, sleepiness) that we are well aware of and believe are being controlled with meds and an appointment for a fluid drain from his stomach area next week.

But in the last few days he has seemingly become more and more confused and his speech has become more slurred. He is more irritable than usual and is literally nodding off to sleep during conversation then waking up seconds later.

A few mornings ago mum made breakfast, he ate it and as mum put the dishes in the sink he asked her what it was they are going to have for breakfast. Just one example of confusion.

Dad and mum are reluctant to call for an ambulance, but honeslty Im not sure if dad even would ask for one, and mum usually follows what dad says. Which leaves kind of stuck with what to do, me and my partner (my partner mostly, he has done a lot of research and feels not enough is being done to help dads quality of life) want to call for an ambulance tomorrow. To have him cared for until he has the drain on his stomach done.

Because dad is tougher than steel I feel every time he sees a doctor (weeks inbetween visits) he tells them hes 'belting' and so they don't get to see how confused he is during the half an hour appointment.

I am very concerned about the last few days, he is more confused, severely fatigued, irritable, not eating much at all. Mum and dad are simply waiting for the next appoinment. Im worried he won't last that long.

Sorry to here about our dad , I had decompensated liver for 3 years before my transplant the only thing i can help you with is the confusion is HE Hepatic Encephalopathy its where the toxins that your liver cant get rid of go into the blood stream and go to you brain it happened to meit makes you all confused I once put the remote control to tele on the back lawn I did not know what i was doing and i started to pass out a lot just for a min or so then they put me on rifaximin tablets and lactulose to make your go to the toilet often to get rid of toxins It made me a lot better but it stopped me falling over I would let them know when you can you dont want to get to the stage of passing out i this helps Lon

I would ring the 111 service if you are worried and they will send out a doctor or advise you what to do if his symptoms get worse. You could always ring his consultants secretary tomorrow as well just to explain his symptoms it might be that if he is already booked in for a drain this week they may see him earlier.

If you are able to go with them it never hurts to have another pair of ears but this will be down to your dad's wishes. You could also ask him if he wants this as this journey is a tough one for the patient and the family carers around him. It's good to talk about the serious stuff when he is in a good place, then you as a family will know his wishes xx sending much love ❤ to your dad and you all xx
