Update to my 10.1 elf

From previous post where dr wouldn't do scan as elf was under 10.5, which nice guidelines do state, but my concern was my drs think anything upto 10.5 is completely normal range with no other references, I did book a private ultrasound last week which showed nothing at all with my liver not even any fatty liver, report said ( liver normal size, outline and echotexture with no obvious focal lesions identified.no evidence of intra or extra hepatic duct dilatation noted.thin walled Well distended gallbladder appear free of calculi.CBD not dilated.head pancreas appears grossly normal, both kidneys outline normally good corticomedullary differentiation, no renal calculi hydronephrosis, normal appearance abdominal aorta and spleen)

I havint a clue what most that means except nothing with liver picked up, but I heard ultrasound can struggle with any fibrosis below advanced.

I have also contacted my Dr for his take on why 10.1 to him is normal and this is his reply or some of it to my email(The normal range for the Enhanced Liver Fibrosis (ELF) test is 0-10.50. Please reassure the patient his ELF score of 10.1 is within the normal range and is not indicative of severe liver fibrosis)

So as you see I was right that they are saying this is normal unless test comes in at 10.51 surely that wrong, this was my only worry before not so much as people said here the Dr won't send for scans as it's below 10 51 which he won't but the references they are using, and as you can see are saying 10.1 my score is actually normal, when it's not, Sorry about the long read.

Hi, But the 10.1 elf means I must have some fibrosis, unlike what the Dr says none? Took advice by the way from your last response changed diet bought multigym and bike feel better in myself, I think sugar and fatty food was making me feel ill, getting up in morning like I had hangover was terrible every day never thought it would be food I eat causing it though, but that's gone since diet change and exercise, been referred to rheumatologist after ANA bloods were raised not sure if there could be somit else causing my raised elf but will see, never seen one before not sure what to expect.

The ELF test is a very specific tool to diagnose severe NAFLD and identify those who are likely to develop cirrhosis from it. That is why there is a straight cut off line, below which you don't have severe fibrosis and aren't staring down the barrel of cirrhosis.

If you want a measure of how much fibrosis you do have, you'll need a different test, usually fibroscan though docs can often gauge it roughly through ultrasound and bloods.

With NAFLD, as long as you are below the severe line, you can entirely reverse any fibrosis through diet and exercise, though I know that's easier said than done. Many docs don't see the need to find out exactly how much fibrosis you have with NAFLD below the line because it isn't clinically important when you can reduce it and track progress with blood tests or ultrasound.

My ELF score is 7.69 described as slightly elevated. I have Severe fatty liver and fibrosis stage 3 to 4. I have 6monthly ultrasound scans and will have a 2nd fibroscan next year. My fibroscan score was 11.6kpa. My ELF score is lower than recommended for screening but my scans confirm otherwise. Everyones case is different, unless other tests rule out the need for further screening maybe push for further tests.

Just to add, in your case your ultrasound showed nothing untoward with your liver.
