Still breathless after 3 years of open heart surgery

end of 2019 I had open heart surgery to repair my mitral valve, close small hole and do ablation for fibrillation.

Not getting proper aftercare or physio after the operation, I had to rely on an app for all that , due to the covid situation. I did my best and after we got back to proper cardio exercise classes at the hospital I have attended those till this day.

My worry is that Iam still getting breathless very quickly , walking and especially up hill, I have to stop to catch my breath several times.

My Dr. got lots of blood tests done and all seems well on that front yet this breathlessness has not really been addressed.

I am 77 years old and have always been very active and this situation causes me stress. I wonder if I should ask to have a lung function test or stress test done.
