I have been taking bisporol for over three years 2.5 feeling fine went cardio said ef 30-35 have no symptoms said have hf everything has been in normal range bp cholesterol eat healthy but since having rampiril sometimes mks me feel tired and dizzy and now hf nurse wants to increase bisporol I ask why said lower heart rate which is only 78bpm and last wk when had mri was only 68bpm my heart is mostly in the 70s so why do I need increase thanks for any info

Hello

If you read the Information Leaflet that comes with the Bisoprolol it gives you a little more information on Why.

My understanding of Bisoprolol is it strengthens the Heart Beat which decreases the Load on the Heart.

Personally I take 7.5 mg daily without any noticeable problems, when I was on 10mg H R went a little to low at 33 BP now it's back up in the lower 60s .

I believe they try to get you on to the Maximum dose you can manage without causing you any distress !!

Yes they’ll likely want you on the max dose that you can handle (Nice guidelines). I’m on Bisprolol but they won’t increase my dose from 2.5 as my heart rate is too low, usually 50s or 60s. I’m on Ramipril too and similar applies, they’re trying to get my dose up from 2.5mg, am trying 5mg but my bp is low when I wake up (this morning it was 75 over 45).

2.5mg is not a particularly large dose of Bisoprolol and I can see why the HF nurse wants to increase this. My GP told me the maximum is 10mg and they try to get you on as much as you can tolerate. I started on 3.75mg and had great difficulty with hypotension, almost passing out when I stood up. I am now on 1.25mg which is right for me as the hypotension is much diminished and my resting pulse rate is around 55. I am intolerant or allergic to many drugs, so getting the right dose was important.

I wonder whether an option for your medication is a small increase in Bisoprolol with a reduction in the dosage of your blood pressure medication Ramipril..... or maybe a change of medication to an Angiotensin Receptor Blocker. Do discuss the options with your HF nurse.

It does seem odd as the increase of bisoprolol will lower your blood pressure further and slow your heart rate which I'd have thought (from a non medical person) is likely to make you more dizzy. Could they not just try you on something other than Rampiril as this seems to have triggered your symptoms? I was on Rampiril which caused side effects - cant remember which as I was tried on a variety of bp meds before settling on candesartan and indapamide. Bisoprolol also causes significant side effects in some people - I found it very hard to tolerate due to tiredness, sleep issues and negative effects on ability to exercise.
