Hi everyone

There seems to be a bit of interest in the hospital form I created. If anyone else would like to look at/use it I’m happy to email it to anyone, just PM me 😉

Word to the wise: it’s a word document so best opened on that - apple products can give it a really odd format 😅

Improvements: my form is ever evolving, but it has been suggested by a consultant at papworth to enlarge the PF score on the last page, and add an area for what percentage that it. I have yet to do this on the template 😅. Of course not everyone has a ‘reliable’ PF so only use/add if that’s appropriate.

Hopefully it’s self explanatory to anyone who uses it, just add/remove anything you feel is/isn’t applicable for you!

I highly recommend this.

Emma, you have done a great job creating this in such an organised format. I believe this should be in the hands of every asthmatic.

I print off my peak flow report from the app I have on my phone when needed and the graph on that is very visual.

I would be very interested in this although I don't have a Windows computer so the odd format might have to do. I guess I could edit it and make it Mac-friendly :)

I'd also saved it from my email into the 'notes' folder on my iPad, but can see no way of editing in order to add the information.

I do use a desktop though, so will save the completed one to the iPad once finished.

I saved my completed one into the ‘notes’ on my phone so that could be your option on phone or iPad once completed?

It seems brilliant! I've been completing mine since yesterday and will print it off when I've spoken to the nurse on Wednesday in case there's anything useful to add to it.

If I'd had this on Saturday, it would have given my husband some useful information to tell the ambulance despatch, as he had no idea of details or particular drugs.

Well worth filling it in as it also has details of any other medication & conditions too!
