Interesting report from John Hopkins University, PRMPete. The five definitions of "medical error" include an unintended act, an act that not achieve its intended outcome, which cover most treatments in medicine by any definition. The failure of a planned action or the use of a wrong plan to achieve an outcome are both euphemistic of medical errors and or tautologous, in that the outcome would be zero. The article notes the systemic problems of diagnostic or clinical mistakes. I would put this down to much more than a clinician's error, rather the co-morbidity and general ageing of patients with chronic diseases.

Do you think that one dodge to get off apportioning blame to the physician is that health wise it was meant to be that patients caused their own misfortune and that the system is not at fault? That might be a useful argument to get off the hook, that medical mistakes are bound to happen whatever the outcomes?

Well PMRPete that made interesting reading, although it was based from figures from across the pond, there's no reason to imagine the statistics here in the UK wouldn't be any less hair-raising .

More than one prescribed prescription pill and I’ve read and heard say that even our Doctors have no idea how our bodies will react or cope with all the side effects many of us will get. (Not saying everyone will get side effects, but many do,)

If they don’t know what all these pills can do to our bodies how come so many are prescribed. ?

Why don’t they check the previous pills to see if it is those that are causing the next lot of presenting medical conditions. ? Rather than add more pills ?

My Mum was on 11 prescription pills which I believe led to her down fall, all have a domino effect on each other, she eventually took herself off of 9 pills and started to feel better in herself, but sadly too late to help her.

She replaced her prescription pills with Vitamins and minerals and turned around a few of her conditions before she eventually passed with kidney failure and an extremely far too late overlooked diagnosis of B12 deficiency a week before she passed.
