Yay! Badgering my GP and presenting bullet points of evidence has delivered a diagnosis. Severe sinus infection, allergies to dust mites, grass pollen and dogs (thankfully not cats!) and minor Bronchiectasis which will hopefully be eased with doxycycline and steroid nasal spray.

great news and a weight of your shoulders I love your cat
She’s a darling cat but with proper tortoiseshell attitude! She was named Pixie by the cat rescue centre because the people who brought her in put her in a plastic box and sealed the lid. The poor animal had nearly asphyxiated so “Pixie” was derived from asphyxia. I guess we both have a history of breathlessness!
