Just looked at this.I gave up because I didn't understand the questions, it wasn't clear if, for instance, you were being asked if you had enough or not enough care. Or is it just me?

Not just you! I started off ok but gave up for the same reason as you, ie - I wasn't sure what I was being asked.

My next question says Have you felt you need support... Feeling more confident in the care you are receiving. They are opposites, it doesn't make sense.
That's was the one that stumped me........I've no idea what it's supposed to mean.
that was very easy to workout. it was worded wrongly.
it should read Have you felt you need more support...to feel more confident in the care you are recieving.

They are not opposite they are two different questions. Post one question at once and I can help you answer. I have been in the needs assessment world since 2008.
I was totally puzzled by it all. It is of no practical help at all.
I took the assessment but was a bit confused if it was about mental or physical health ?x
hI these forms are tricky so it's best to get help to fill them in from either CAB or a local disability group.
The thing to remember is if you can do something say yes but also say it takes me 2 hours and I am knackered for days afterwards. The criteria is if you can do something in a timely and painfree manner. If not it's important you tell them.

Hi I got help from a social worker and he said always answer the question if it was your worst day as they take it that there is no problems if you give the answer they are looking for
I think it was just the question I quoted that couldn't be answered. However, the last one has no options so can't be answered. Basically the whole thing is poorly worded and the answer options are not always appropriate. I have no idea what the survey is supposed to achieve. I have no interest in suggesting corrections, I am sure whoever generated it can manage that themselves.
Needs assessments are complicated to fill in if you have never done one before. Basically it is asking do you need any more care? Normally if you are completing the needs assessment - you do need more care.
The bad news is once you have completed one needs assessment you tend to have to do one a year.
