Apologies if this has already been discussed (if you could bump up a previous thread I wod appreciate it) but does anybody have an information sheet that they take to A&E with all of the important bits on? Am thinking about devising one for myself so that when I am unable to talk in sentances can just give it to the guys in A&E. If you have one, what is on it?
Am greatful for any suggestions

I make my own.
I just get a small piece of card and fold it in half.
On the front i write my name, address, DOB, Dr's name and address, Cons name and address.
On inside i write my medications currently taking and in what form eg nebs, tablets, inhaler etc and any allergies i have(Written in RED)
Then on the back ii put next of kin name, address and phone no.
This way i can update at any time if any med changes at all.
I carry one in my purse, in my bag and i have one in my med box at home.
I always make sure that my friends and family (including kids) know where to find them.

Hi, i was given oneby my consulant. It has my name D.O.B contact numbers ect. it has all my medications. It also tells the docs in a and e what 2 do e.g whats nebs and ivs i need and when. It is realy useful.

There is a really good medical record card that you can personalise and print off on the doctorbabel website. It has been put together by an A&E consultant to make sure that, in an emergency, the doctors are aware of medications etc. You can also translate it online (free of charge) if you are going on holiday. I have one and found it really useful recently when I had an outpatient appointment and needed a list of all meds.

i also wrote my own and print it on bright yellow paper that way it stands out in my wallet or bag and med staff are less likly to loose it. my local cons hasnt really been that keen me having this as he said it stops the doctors thinking as they just follow the plan rather than use their clinical judgement, but quite franckly if i am in a real big go off i dont care, if i am not so bad and can talk it is not so necessary to use it.it is also helpfull if i am not in local hossie. local hossie does have a plan for me written by resp nurse, reciently a different hossie did call local for advise, a first!
