Flecainide will be prescribed by a Doctor to treat a patients specific treatment needs and these will obviously vary. Before my ablation, I was taking a low maintenance dose of 2 x 50 mg per day. Some may take 2 x 100 mg per day but it is important not to exceed 300 mg within any 24 period. Flecainide is a potent drug and it’s important not to change doses without first consulting your Doctor.

I take Flecainide and because my episodes always happen at night I take 50mg in the morning and 100mg at night. This works for me and the changes were made following discussion and advice from my EP. I agree you need to find what works for you but you should do this in consultation with your EP/Cardiologist. Good luck!!

I take Flecainide twice a day, 100mg in the morning and 50mg at night. Taking a dose of 100mg at night gave me hallucinations, so had to cut down.

Ditto, except for the hallucinations....and always taken on an empty stomach.

I thought that when you go into permanent AF, you take rate control drugs and not anti-arrhythmic drugs. Also, I read on this forum that some in permanent are only on an anti-coagulant. Is there a reason to still take an anti-arrhythmic drug, though permanent?

I have never heard what you're saying. However I think a lot of EP's/Cardiologists have different ideas.

Yes, I agree these specialists have different methods. My conclusions were not based on trials. They were based on noting what different members on this forum were taking or not. I will now keep a list and see what sort of pattern comes up.

I was taking 50mg of flecainide in the morning and 100mg at night but find side effects in the evening so I started taking 50mg at night...feel better on this dosage.

I now take 100mg Flec in the morning and 75mg at night, but right after taking the night dose I feel my heart play up for about half an hour - just aware of the strong beat. I've learnt over the 16 years of having AF that the less I think about what it's doing the more likely it is to calm down.

Thank you for your reply.Did you have an ablation?

jeanjeannie50 profile image

Have had three ablations, but now in constant AF which is nowhere near as bad as PAF.

What is heart rate when your in afib?Sorry remind me what is PAF?

jeanjeannie50 profile image
jeanjeannie50 in reply tofrancesturpin

PAF is paroxysmal atrial fibrillation- that is AF that comes and goes, you can go days, weeks, months or years in between attacks. At times my heart rate would go too fast for me to record probably over 200bpm. I would often end up being admitted to hospital. Now I feel fairly normal heart-wise. Just get some days where I tire quickly. My EP said some people are not helped by ablations and I'm one of them. I changed my diet drastically and am so much better for doing that. Never eat large meals or food containing artificial additives now.

That is the dosage I'm on...I had an ablation for flutter about six months ago. Which was successful. They discovered I have afib as well. I do not want another ablation so I hope the flecainide works for me as it has done for you. How often do you see your cardiologist?

Thanks:)

I take 2x50mgs daily and have been on them since diagnosed 12yrs ago. I've only had one short episode since then so I guess they're doing their job.

100mg twice a day, keeps my AF at bay.

100mg in the morning, 50 at night, rarely get any AF and no side effects of note.

I started at 50mg x 2 a day, then moved to 100mg X 2 a day, then after 2 years went to 150mg X 2 a day. I kept on 150mg X 2 a day for 10 years, after which I was found to be in asymptomatic persistent AF. (I am 6'7" tall).

Not only are we all different but so can the advice be. My cardiologist put me on x2 100mgs per day. As my AF had always started late afternoon/night I questioned whether it would be better to take less in the morning but his response was 'he favoured a smooth approach rather than a rollercoaster for the heart'. So far approaching 7 yrs this has worked but of course the alternative might have worked as well.

100mg twice a day - working well so far. I can take an extra 100mg a day if afib returns as a PIP (300mg a day max).

Best taken on an empty stomach - not after a big meal !

There are a wide range of answers. In addition, for some/many, the answers will change with time, both up and down. With flecainide there is a particular danger that increasing the dosage can increase heart irregularities, so I think it worth repeating that changes in dose should be done in discussion with someone competent who is able to act if things go wrong.

It is usually taken twice a day, unless you take the slow release capsules. This is because of its properties .. half life etc.

I take 100mg morning and night & have for only 3months, after my third cardioversion. I’d firstly been in persistent AF (asymptomatic) for an assumed good while & it left a bit of a smoking wreck behind so I couldn’t have Flec until I’d repaired a lot. Thankfully that has happened and I have everything crossed that it continues to work. I do miss a few pints now & again with this AF since diagnosis - BUT the Flec has had an upside for me, I’ve been having some absolutely bonkers dreams & get up in the morning chuckling quite often. It really has added some entertainment - as good as side effects get I guess?
