abiraterone+predni failed after 2 yrs 10 months. Had SBRT and VMAT 33 sessions one year ago to swat L1 lesion .. pSA gradually rising ,now at 6.7.

what can I do as a next step to fight the beast. Any recoms? Anyone have the same situation?

Hello,

I was faced with the same dilemma last January. Both the ADT drugs and the ARPI drugs I was taking were no longer working. Scans were showing increased activity and my PSA was rising. I now had to make the move to cytotoxic drugs that kill cancer cells as opposed to putting them to sleep. Three major choices are chemotherapy, immunotherapy, and radiopharmaceuticals like Lutetium (beta) and Actinium (alpha). I chose the last option and so far it is working. All three have their advocates and their detractors. Do your homework, see what your oncologist recommends, and good luck!

But to take radiopharmaceuticals like lutetium I thought I understood that to receive them you had to have done chemo first, or am I wrong?

Hello,

I was on a clinical trial when I received Lutetium so I didn’t have to do chemotherapy first but (as the result of several successful trials) they are relaxing the rule of having to take chemotherapy first. Good luck!

The important thing is not to immediately go on drugs similar to Zytiga such as Xtandi Erleada. Better to do chemo now and after chemo go on Xtandi or Erleada. Many after the failure of the first new generation hormonal drug given the few side effects want to immediately try another, but with poor results (they are very similar drugs there is a strong cross resistance so they should not be used in sequence and better to alternate them with chemo, doing so they should work longer). Furthermore there are testimonies and a study (which TA showed me some time ago) that by replacing prednisone with dexamethasone you can extend the effectiveness time of Zytiga. Talk to your oncologist about it
