I get Lupron every three months. I will be out of town when my next shot is due. The pharmacy told me to ask my MO when I would get it but that could take too long. I don’t think a week earlier or later would make any difference ( especially because I have been on it for 6+ years) but what does the collective think or what have you been advised in the past? I think a week late is OK.

Earlier is better than later.
I think a week late would be fine, especially given how long you've been on it. Trust me, if you stopped altogether it would be months or years before your testosterone came back. I'm going through that now.
If the injection occurs too soon following the previous one insurance may not cover the cost. My onc said the manufacturer guarantees effectiveness for three months (in your case) but it will very likely maintain strength for a few weeks longer, if delay is necessary. Current T level is one thing to consider in this decision. 20-50 is different than <5, for this purpose.
In my seventh year on Lupron (every 4 month injection) I was late by more than a month. Nothing happened to my PSA. Now in 11th year but make sure I get them on time.
