Hi everyone,I'm busy going though Lu177 infusions, 2 weeks after the 2nd round and my first psma scan on the 20th August, I have low psa even though I have progression and my doctor is concerned its not working,I can only assume this is because of the creeping up of psa, my question is where do I go if the 177 isn't working?? In this country (Australia) it is very expensive so if it does not work do not proceed with it. I am not a doctor but that is what I did anyway & went back to ADT.

MO said he still had options if Pluvicto didn’t work-carboplatin being one chemo. Radiologist said Xofigo was an option. Actinium is being also tested and available in clinical trials.

Does low PSA correlate with low PSMA? It's the PSMA antigen that Pluvicto wants to stick to. Did they do a PSMA PET scan before starting treatment to assess the potential for uptake of Pluvicto. Most of the other radioligand treatments in early testing currently rely on PSMA as well. There is one out of Australia by Clarity using Cu-67 that seems less sensitive to low PSMA. The Actinium based treatments all look interesting. There is also an Astatine based treatment being worked on in Japan that looks good as well. Lots of early trials that are moving from phase I to phase II. So maybe you can get into one of the phase II trials.

Hi no my pet psma showed very avid uptake, which is why they put forward 177,my psa has never been crazy high, 10.7 when diagnosed,and has never gone higher than that, a jump of 3 on the first round of 177 for me is a big jump, my doctor seems to be waiting for a large spike then a drop?? I have requested to be on the actinium trial at the royal marsden if the 177 doesn't work,I am eligible for it.

after 4 rounds this treatment was stopped. Many new Mets in skeleton and higher PSA. He is now doing chemo - Jevtana - PSA has gone from 235-49 after 5 rounds

What SE’s have you had from the LU-177? I had dose one this past Tuesday. I was knocked out for 3 days and still feel unwell, and have intense breakthrough pain.

Hi sorry for delay in replying, I had nothing on the first round apart from pain in my legs,round 2 nausea and vomiting and the pain was bad enough to have orimorph and fatigue, psa still rising big scan yesterday report to follow
