Question as to next steps. My details are up to date in bio and I was undetectable until last week when I hit 0.021. G-9, t3B, PNI, IDC-P & lymphovascular invasion with decipher of .41. I live in Sarasota most of the time but Milwaukee was/is still home due to work. I shuttle back and forth every 4-5 weeks up north for a one week stay and to see our son as well. I like my RO Dr Colleen Lawton & Dr Deepak Kilari MO in Milwaukee my guess is that they will want to radiate sooner than later which is fine. So in the SRQ airport I see Dattoli signs all over. He claims to be world renowned with his DART technology & I have a phone consult scheduled for next week but I am skeptical due needing super expensive equipment that only hospitals typically can afford. Plus every few years you need to update the multi-million $$$ equipment. In Sarasota i also heard that Dr Liz Guancial MO is excellent as well for prostate but not sure of their radiology department. I have Moffitt Dr Grass RO who is only 90 minutes north. Regardless of location I am willing to travel to try for best results.
So 2 questions;
1) typically how long does one wait before radiation 0.05 or 0.1 or? Is 0.021 too low to even think about it yet?
2) What type of radiation is typical for salvage radiation?

Every month will include large amounts of natural variance and lead to erroneous conclusions. Once every 3 months is more than adequate. There is nothing to be done unless it reaches 0.2.
Thanks! I was reading my pathology and paranoia set in because RO told me 3 years ago if Decipher would not be .41 he would have recommended radiation almost immediately after surgery but again with .41 he said wait
I could be wrong (my wife will tell ya) but at some places undetectable PSA could be <0.04. Maybe even <0.05. So your small detection could be something but might be noise. Easy for me to not worry, you not so much. Repeat tests will give you a trend.
Peppertree602, my husband and I considered Dr. Dattoli for RT after BCR from both a simple and a salvage prostatectomy. Together, we decided against seeing him because his insistent plan was to do brachy and EBRT on a patient who had no prostate. He insisted that there was tissue where he could do brachy. Dr. Vipul Patel (robotic surgeon in Celebration) Celebration Hospital pathology, and Mayo Rochester pathology said there was no tissue available where brachy could be used. Dattoli's plan was unwavering so we left. Just a personal experience, nothing more.
I have heard very good things about Guancial as well but, like you, know nothing about the radiology department.
You're welcome. We had a short term rental arranged for 2 months in Sarasota. We did an about-face and went to Dr. Matthew Biaggoli (then in Orlando with Advent Health, now in Sarasota) instead. We were most happy.
Google Dattoli arrests, between prescription and insurance fraud charges I cancelled my consultation appointment early on. I see Dr Guancial, wouldn't trade her for anyone, but have never had radiation treatments you seek, my cancer was too far along when discovered. Doing Pluvicto now.
I had nearly identical pathology to yours except 4+3. However I had a positive node, so there’s that. I had subsequent RT after RP in 2019, with abiraterone and 18 months ADT. Undetectable and in fine health 4 1/2 years later.
Between your bio and this post I would say your cancer is nothing to worry about compared to your anxiety. 0.021 is cause to celebrate (or more accurately, to continue celebrating) until further notice.
My only real advice: Keep one eye on it only, and trade in your glut of supplements for more exercise. Supplements help somewhat and sometimes. Exercise helps better and always.

Prominent Sarasota cancer doctor charged with prescription and insurance fraud, police say

SARASOTA, Fla. (WFLA) – A prominent Sarasota doctor is under arrest accused of calling in fraudulent prescriptions for valium.
Investigators with the Sarasota Police Department say Dr. Michael Dattoli’s prescriptions were issued under the name of one of Dattoli’s former coworkers at the Datolli Cancer Center on behalf of Dattoli’s wife.
A former employee of the Datolli Cancer Center told Sarasota police officers that during a check of the Florida prescription drug controlled substance database in September 2021, he noticed several fraudulent prescriptions were written in 2020.
Three of the fraudulent prescriptions were written to Dattoli’s wife who was never the employee’s patient.
The victim was shown a copy of the phoned-in prescriptions during his interview with Sarasota police and said he never authorized the prescription.
Dattoli was charged with three counts of obtaining a controlled substance by fraud, three counts of criminal use of personal identification information, and three counts of insurance fraud.
He is currently being held at the Sarasota County Jail.
Don't see Dr. Dattoli for 2 reasons. One he is not an expert on anything and 2, he was charged with fraud for writing prescriptions.Start with Dr.Barzell in
Sarasota if you want a good one. I had a double prostate cancer problem but
i talked with Dr. Datolli and his partner each in separate phone calls. I found them both to be very competent as far as i could tell.
I communicated by forum and email with a man that was treated there maybe 15 yrs ago and he said they were light years ahead of the other places he was at.
My phone calls were exactly at the time of his legal troubles. Someone was writing prescriptions using his prescription pad. Maybe his ex-wife or family? I can’t remember.
