For me by the second infusion. It just did not turn out to be durable for me. Lasted 6 months. PSA started going up.
After 3 to 4 weeks. This article has info about temporal changes of the PSA during Lu 177 PSMA treatment and outcome.
My husbands started to drop 4 weeks after the first infusion and has now dropped significantly. Unfortunately there has been significant spread of the tumours throughout his skeleton so the treatment has been stopped after the 3rd infusion despite the PSA continuing to fall.
Do you know what's the cause of the spread? And why would Pluvicto be stopped if PSA is dropping?

They don’t believe it’s working and don’t understand why the PSA has dropped. We are waiting for the bone scan results and the next PSA test in 2 weeks then we are going to discuss further as my husband wants to continue. That said he is getting more pain flares this time.
How did they find the new tumors? PSMA Scans? Or bone scan?
I have had 4 infusions so far. Blood test three weeks after first infusion PSA started to drop and continued to drop. Before first infusion PSA was 68.37 three weeks after fourth infusion PSA was 9. Will have PSMA-PET today, next week blood test and 5th infusion.
Thanks. My after first infusion PSA is at 419. Hoping for a drop next time.
In my case it took just before my 3rd treatment to drop from 62 to 34 Oncologist super happy, said they are seeing a 'delayed response' in some people. Hang in there Russ.

Pluvicto failed for me from the beginning. I had a PSA of 1.56 at the beginning of treatment and after the 3rd my PSA reading was 10.18 I stopped the treatment after that. Now, that doesn't mean it won't work for anybody else, chances are it will. It just didn't for me. I have exhausted all other treatments. I'm going on 10 years with this disease now. I have to rely on clinical trials now when available. Until then, I'm back on Jevtana which kept my PSA between 1-2.
