It can. There are several relatively rare types of PCa that don't express much PSA. A periodic scan can give assurance, particularly when there are symptoms (like pain). For most men, PSA and bone-specific alkaline phosphatase are fine.

I had a different but related question that my RO would not answer; while on ADT can PCa cells spread, migrate, populate and wait for the ability to start growing again once ADT is suspended...I could not find anyone, or any research, to answer this. My dilemma was that I had bladder neck invasion and clear margins/ vesicles/ lymph nodes, etc. I was biochemically recurrent right out of the operating room; PSA 0.13 at 6 weeks. I immediately went on ADT for 6 months to heal, then pulled the trigger on salvage RT because I could not get anyone to answer my questions...
For me (an engineer) I had cancer cells on my bladder that had been 'incased' between the prostate and the bladder, that were now flapping in the wind...awash in blood and fluids and whatever happens in those areas after surgery. Soooooooo, why would these cells not be able to sluff off, migrate and move about WHILE they were suppressed by ADT treatment? Again, could not get anyone to tell me they would not...my RO did tell me that ADT did not kill the cells, so you can draw the lines here and make your own conclusions.

And when entering the advanced arena, PSA isn't treated, the cancer is. PSA might be an indicator that something is active, but it doesn't tell the entire story. Is why other testing is involved as PSA "alone" is not a good diagnostic or prognosticator...

Yes. Some people even have aggressive forms of PCa where their PSA is in the normal range.
Indeed it can. My PSA was 2.3 when mine was discovered by digital exam while I was at the urologists for other issues. While that was actually high for me (I was typically below 1), to the urologist who didn't have my prior numbers, it wasn't yelling PCa at him.
