When it hits the bones...

I was dx. in '09, surgery in early 2010 (confirmed G4+3). Failed. SRT in 3/2011. Completely failed (PSA never slowed down). Since then, HT which works great and allows me to take HT vacations. When PSA comes back: scans. Did a C11 scan at Mayo in 2014. Found in external lymph nodes. Did SBRT (and HT for 3-6 mos). Gave me time. PSA reared it's ugly head a few years later. Did GA-68 PSMA scan at Umich. Similar result. Same exact tx. It worked till very recently. Will do pylarify PSMA scan at UMICH next month. My questions: Say my PSA is about 2-2.30.

1)What are they likely to find at this level (is there even an answer?)

2) Suppose they find a few lesions in the bone. Do they also do SBRT to obliterate those spots like they did in my ext. lymph nodes. Or do they just wait until/if pain/discomfort arises and then try to palliate this?

They will probably find mets at this PSA level. But it could just be lymph node mets. I think this is more likely.

If they find just about three bone mets you can get them radiated with SBRT. It depends whether you insist on getting them radiated.

There are anecdotal accounts of people playing whack-a-mole for a long time. You probably have to say it hurts to get insurance to pay. And of course everyone's cancer has a different aggression level.
