Anyone on or done the triplet ?

How is it working out for you ?

I’ve been offered the opportunity by paying for the docetaxal and as a low volume man is it still as beneficial as high volume ?

Any feedback greatly appreciated

The only triplet that has been proven beneficial in all men is ADT+chemo+darolutamide (ARASENS). They have not published separate results for men with low volume or high volume of metastases yet. The triplet of ADT+chemo+abiraterone (PEACE1) has only proven beneficial in men with high volume of metastases so far. This doesn't prove that a triplet won't be beneficial in men with low volume of metastases, it just takes longer to prove a benefit, if there is one, because men with low volume of metastases live so long.

Genomics may prove to be a better guide than counting # of metastases. A Decipher Grid Analysis (on the biopsy cores) or detection of genomic mutations in PT53, WNT, and DNA-double strand break repair genes may turn out to be a preferable indicator of potential benefit.

My husband is about to start triplet therapy in 3 weeks. He has de novo low volume disease. He he is on ADT and will have 6 rounds of Docetaxel and Daralutomide. Then he will have radiotherapy to prostate and any lymph nodes and small bone met after a couple of months. Off the back of the Peace and Arasens and Peace Trials his oncologist wants to him hard with intense early treatment. He is 50 and fit.

That is a great plan! Who is his oncologist? Did he get insurance approval?

We live near London. My husband has health insurance and I realise we are fortunate to have more options. Everything has been a steep learning curve since husbands shock diagnosis ( found randomly in health check, no symptoms). I found this site while looking up as much information as I could to help him.

Usually, the Docetaxal is prescribed in the high volume disease. It has been found to prolong survival and improves a range of outcome measures, including time to metastatic progression and subsequent therapy. There is a significant paucity of published data in the literature, in patients with low volume metastatic prostate cancer and Docetaxal is associated with significant side effects. It would be advisable to have a detailed discussion with your treating physician regarding the usefulness of Docetaxel in your scenario.
