SBRT failure ?

Hello All , Im new to this great site . I had SBRT in 2016. My PSa levels have hovered in the 1.2--1.6--1.06---1.7 range . My nadir was .83 in 2018 . So my Uro gave me a uPSA (ultrasensitive psa) recently and that was 3.07. Areal shocker . So i took another regular PSA 1 month later and that was up to 2.9 from the 1.7 I had 3 months earlier . Its doubled . So my RO gave me a script for the PSMA-PET that I will be taking this tuesday at VA , Hines , I guess the horse is out of the barn . . wanting to say hello to Tall Allen as we spoke often while at HealingWell . My husband had 5 sessions of SBRT on a met in the left cervical C7 in October 2020 but it didn't work. His PSA continued to grow and the Ga68psma petscan showed that this met is still very aggressive. So we are now in Germany for a second Lu177 psma treatment next Tuesday, December 7, after the first injection in October 12. The PSA yet dropped from 6.14 to 5.38 and now it was 4.60 before the second shot. Hopefully this met will disappear soon. My husband underwent a RP in November 1018, is Gleason 9 and stage 4. He had no ADT and is still hormone sensitive. After the RP his PSA was 2.16 and in 2019 he's got 3 Lu177 injections in Austria. Until April 2020 the PSA dropped to 0.31 but then climbed to 1.22 before the SBRT in October 2020. After the SBRT it continued to rise quickly : from 2.72 in May 15th 2021 to 6.14 in October 11th 2021. He has four or five other small mets but less aggressive. We will see the results from the second treatment on the next Ga68psma petscan and also the PSA in the next months.

Welcome to this forum, JJ. I'm sorry for the biochemical recurrence. The PSMA will tell you where it is. It may involve as little as some focal salvage brachytherapy. Please write again when you get the results.

Thanks Tall one , You mention focal salvage brachy ? What would that be if needed , SBRT or HDB as done with Dr. Martinez ? After the scans on tuesday should I expect ADT in some form ?. Im in Chicago area and my RO Dr. Arica Hirsch that done my SBRT referred me to a Dr. Russel Z Szmulewits at the University of Chicago ? My PSMA is being done at V.A. since this cancer is Agent Orange connected . Im not shure of their other protocal as far as good,better , or best . I do know they have staff from Loyola University that does HDB . So we will see . Thanks Allen

Martinez can certainly do it. But wait and see what the PSMA PET scan shows you first. It can show a lot of different things. Stay in the present if you can,
