Diagnosing and managing radiation cystitis

I underwent SBRT about 3 months ago and now experience urinary frequency, urgency, nocturia, flank pain and penile discomfort. Urine culture and CT are negative.

Looking to see if anyone has had a similar experience? What diagnostic steps were taken? Was hyperbaric oxygen considered? Other options?

With ViewRay SBRP using MRI guidance, I was hoping for a lower incidence of cystitis. I can’t be sure it is due to the radiation, but if so, I hope the symptom is short lived.

Those are common symptoms, and usually transient. I took NSAIDs and Rapaflo. If you are having bladder spasms, Myrbetriq may help. If, after a few months it hasn't resolved, I think HBO is a good idea.

Do any studies indicate that such problems may be less frequent with 20-40 session IMRT, when compared to 5 session SBRT? Any reliable data/studies indicating frequency of such toxicities lasting longer than x months?

This is a great study. The article addressing the biological tissue effect is important to understand the potential benefits of SBRT. I was impressed by the ongoing MRI monitoring with ViewRay which provides live beam adjustment or termination with displacement of the prostate.

Thanks Allen! I kinda understood that from some other stuff I'd read, but your tables are very clear in the comparisons. I'll search your articles for some comparisons re RP. It is easy to fall into the trap of anecdotal evidence we read in these forums!!

wow, old post, but got my attention becuase those problems are what the Kasiser urologist described as primary urinary problems with RT!! Supposedly dissipate with time..hope yours did!!!
